This tool is intended for mandated reporters to submit non-urgent reports of elder or dependent adult abuse to San Diego County Adult Protective Services.  Do not report abuse of children which is done through separate portal

IMPORTANT INFORMATION TO INCLUDE IN REPORT AS A MANDATED REPORTER

  • Your name, agency, and contact information (you are the reporting party; include anyone who provided information as collateral contacts).
  • Accurate demographic details – APS is unable to follow-up without an accurate address.
  • In the “What happened today that led you to make a report section” include:
    • Full narrative describing who, what, where, when, and why.
    • Explain why reporting now if incident is historical.
    • Include the current safety level of the person (1 = most unsafe, 10 = most safe);
    • Has the abuse been mitigated in any way prior to this report.
    • Whether the person/client can advocate for themselves and if not, describe the limitations.
    • Any law enforcement event or report numbers.
  • Any agencies or people involved, their role and contact information in the “Other Persons Believed to Have Knowledge of Abuse” section (check the add person box).
